If you are going home after a stay in hospital or are going into hospital as a planned admission we can help you with advice and practical solutions which may help you manage more independently when you go home. We can visit you in hospital or at home either before a planned admission or after discharge.
We also offer information and training to health care professionals who are involved in managing hospital release.
